HOMECARE COORDINATOR POSITION DESCRIPTION
POSITION PURPOSE
To manage the delivery of Home Care Services to participants, ensuring that they receive the support they require to remain living independently in their own homes.
PERFORMANCE OBJECTIVES
Your main responsibilities as a Homecare Coordinator include assessing participants' needs, developing individual planning documents and coordinating the delivery of services by a team of support workers. You will also monitor participants’ progress and adjust individual planning documents as needed to ensure that participants are receiving the appropriate level of support.
As a coordinator, you will be required to conduct a leadership role within the team of support workers through the provision of supervision and skills development.
Key Responsibilities:
· Conducting individual planning and completing appropriate documentation to ensure participant’s needs are met and they are supported to achieve their goals
· Identifying the training needs of support workers and ensuring all support workers understand and are trained in individual support plans
· Supervising staff and providing ongoing feedback, support and addressing any performance issues
· Updating individual support plans, risk assessments and other planning documents in line with participant’s changing needs and circumstances(minimum annually)
· Ensuring you have a physical presence within the homes to monitor the support being provided
· Fortnighly reporting to the Director
Detailed Responsibilities of the Role:
· Conduct assessments of participants’ needs and develop individual planning documents that are tailored to their individual circumstances
· Monitor the participants’ progress and adjust planning documents as needed to ensure that they are receiving support that meets their needs, goals and preferences
· Ensure participants are able to select their preferred workers, including gender, and to work with the rostering team to ensure that these preferences are respected.
· Ensure that services are delivered in compliance with relevant legislation, policies and procedures
· Provide support to clients and their families, answer questions and address concerns
· Complete vulnerability check-ins with participants in accordance with their vulnerability rating and document outcome within progress notes.
· Ensure support workers receive the training and support they need to perform their jobs effectively
· Monitor Behaviour Support Plans and report weekly to Director
· Communicate effectively with Care Team including participating in Care Team meetings where required
· Report staffing and conduct concerns to HR
· Undertake supervision with allocated staff members
· Conduct performance appraisals with allocated staff members
· Review progress notes to ensure accuracy and completeness
Screening Requirements:
· 100 points of ID.
· Right to Work in Australia.
· NDIS Worker Screening Check.
· An international police check (where applicable).
· Working with Children Check.
Key qualifications, skills and competencies:
· NDIS Worker Orientation Module Certificate.
· Infection Control Training Certificate.
· Relevant degree in disability, mental health, or aged care (desired).
· Demonstrated experience in team leadership.
· Thorough understanding of person-centred support and best practice approaches in the disability sector.
