About the role
- Research both NDIS funded and unfunded services and supports which will enable our clients to achieve their goals within their NDIS plan.
- Monitor plan outcomes and expenditure to ensure the services remain within the plan budget.
- Strengthen and enhance NDIS participant's ability to coordinate supports and self-direct their services in the longer term.
- Build or support the capacity of clients to develop relationships and partnerships in local communities.
- Ensure mainstream services (i.e. housing, education, justice, health) and NDIS paid supports to meet their service obligations to NDIS participant.
- Provide our NDIS participant and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequency.
- Clear and accurate record keeping of information.
- Identify and pursue new business opportunities across existing and emerging markets.
- Coordinate networking activities, industry events, and client engagement initiatives.
- Engage with therapists, allied health professionals, and referral partners to generate new support coordinator client

Skills and experience
We require passionate, experienced and versatile Support Coordinators able to:
- Demonstrate an understanding of the NDIS requirements
- Demonstrated experience in working the Disability, Aged, Mental Health or Nursing sector
- Preferable 6 months of experience working as NDIS Support Coordinator
- Provide a current DCSI or NDIS Worker Check (or willing to obtain)
- Working with children check (or willing to obtain)
- Be self-motivated with the ability to work autonomously and as part of a successful team
- High level verbal and written communication skills.
- Experience working within a holistic practice framework with other providers/supports to achieve the best outcomes for clients.
- Flexible/hybrid working to enhance your work/life balance
- High level computer literacy with proficiency in word processing, Outlook and including high level key board skills.
All customers receive services in the community, in their own homes, supported accommodation and residential care. Support Coordinators are expected to spend a significant time on the road attending home visits, team meetings or other appointments.
