About the Role

As a Specialist Support Coordinator, you will focus on providing person-centered support to individuals with high or complex needs within the NDIS framework. Your role empowers participants through a "do with, not for" approach, increasing participants ability to manage their own supports.

Key responsibilities include developing personalised support plans and working within funding budgets. You will build strong relationships with service providers and stakeholders to ensure appropriate service delivery while addressing complex barriers to access. You will offer expert guidance on utilising NDIS plans and ensure consistent support during crises. As a Specialist Support Coordinator, you will meet reporting requirements to the NDIA, guaranteeing transparency and accountability in service provision.

About You

You are a compassionate and organised professional with proven experience in complex case management or specialist support coordination, particularly with individuals facing complex needs, including disabilities and mental health challenges. Your strong understanding of person-centered approaches and commitment to inclusion enable you to connect empathetically with participants, ensuring their needs are met.

With excellent time management skills, you prioritise tasks effectively and thrive in independent work environments, demonstrating resourcefulness and problem-solving abilities. You have a deep commitment to cultural awareness and inclusivity, particularly in your interactions with diverse populations, including LGBTIQA+ individuals and Aboriginal and Torres Strait Islander peoples.

You hold a relevant tertiary qualification in an Allied Health discipline, such as Social Work, Developmental Education, Rehabilitation Counselling, or Occupational Therapy. You have demonstrated the ability to create person-centred support plans, helping participants develop and progress toward their goals. Your strong knowledge of community networks and services allows you to link participants to suitable providers, ensuring their needs are met.

You are proficient in Microsoft Office and can work in cloud-based software, making it easy for you to document and report on the services you provide. You maintain high standards in documentation and reporting. You also understand the regulatory landscape and can liaise with the NDIA and NDIS Quality and Safeguards Commission as needed.

With integrity and transparency guiding your work, you build positive relationships with stakeholders and work collaboratively to deliver high-quality support. Your passion for empowering others and your trauma-informed, strength-based approach makes you an ideal fit for this role.

Requirements:

  • A South Australian Driver’s Licence
  • Current DHS Working with Children Check (WWCC)
  • Current Police Clearence 
  • NDIS Worker Screening Check
  • Tertiary qualification in a relevant Allied Health discipline (Social Work, Developmental Educator, Rehabilitation Counselling, Occupational Therapy)

Location

This role is based at Bowden just a short walk to the popular Plant 3 and Plant 4 entertainment complex in Gibson Street however some off-site work will be required.
