Are you passionate about supporting people with a disability to live more independent and fulfilling lives? SA Disability Pty Ltd is currently seeking a part-time Support Coordinator to join our growing team in Kent Town, South Australia.
As a Support Coordinator at SA Disability, you will deliver high-quality, person-centred support to NDIS participants with high or complex needs, including psychosocial and multiple support requirements.
You will work using a “working with them, not for them” approach, empowering participants to build capacity, strengthen independence, and confidently manage their supports and NIDS goals.
This role requires advanced coordination skills, sound judgement, and the ability to navigate complex service systems while ensuring participants receive consistent, ethical, and outcomes-focused support.
Key Responsibilities
- Provide support coordination to participants with normal to complex and high-risk needs
- Develop and implement individualised, person-centred support plans within NDIS funding parameters
- Build and maintain strong relationships with service providers, stakeholders, and informal supports
- Address complex barriers to service access and resolve system challenges
- Provide expert guidance on NDIS plan utilisation and participant choice and control
- Support participants during periods of crisis, transition, or change
- Meet NDIA reporting requirements, ensuring transparency and accountability
- Maintain accurate, timely documentation aligned with NDIS Practice StandardsWhat you'll be doing
- Develop and implement tailored support plans for clients with a disability, based on their unique needs and preferences
- Facilitate access to a wide range of community services, aids and equipment to enhance clients' independence and quality of life
- Coordinate the delivery of supports, monitor progress and make adjustments as required
- Advocate on behalf of clients to ensure their needs are met and their rights are upheld
- Maintain detailed records and prepare reports to track client outcomes
- Build and maintain strong relationships with clients, their families, service providers and other key stakeholders
- Confidence liaising with the NDIA and NDIS Quality & Safeguards Commission when required
- Strong cultural awareness and commitment to inclusion, including working with
- Aboriginal and Torres Strait Islander peoples
- LGBTIQA+ communities
What we're looking for
- Relevant tertiary qualification in a human services field, such as Social Work, Disability Studies or Community Services
- Minimum 2 years' experience in a support coordination, case management or similar role
- Demonstrated understanding of the National Disability Insurance Scheme (NDIS) and experience navigating the disability support system
- Excellent communication and interpersonal skills, with the ability to empathise and advocate effectively
- Strong organisational and time management skills, with a solutions-focused approach
- Willingness to travel within the local area to meet with clients
- Current driver's licence and access to a reliable vehicle
