About us:


AQA was established in 1987 by a group of people with quadriplegia who saw a need to improve access to information, support and employment opportunities for people who had sustained a spinal cord injury (SCI) – particularly quadriplegia. Today AQA continues to provide individualised supports to people with spinal cord injury and other physical disabilities.

About the role:


We are seeking a compassionate and motivated Support Coordinator to join our team based in Fairfield.

In this role, you will work closely with participants of the NDIS to help them navigate their plans and access the support services they need to achieve their goals.

You will collaborate with service providers, create tailored plans, and ensure that each participant’s needs are met.

What your days will look like:


Assist NDIS participants to understand and implement their NDIS plans.
Coordinate and manage the delivery of support services in line with individual goals.
Liaise with service providers, families, and other stakeholders to ensure high-quality service delivery.
Monitor and review progress towards goals, adjusting plans as needed.
Advocate on behalf of clients, ensuring they have access to appropriate services and supports.
Provide information, advice, and guidance to participants and their families regarding NDIS processes and funding.
Maintain accurate and up-to-date records, ensuring compliance with NDIS regulations.
Work collaboratively with participants, their NDIS, medical, and mainstream supports to maintain ongoing supports and connection.

About you:

Previous experience in disability services, community services, or a related field as a Support Coordinator.
Knowledge of the NDIS process and its impact on individuals with disabilities.
Strong communication, interpersonal, and problem-solving skills.
Ability to work independently, manage competing priorities, and meet deadlines.
A commitment to promoting and respecting the rights and dignity of individuals with disabilities.
Tertiary qualifications in Social Work, Community Services, or a related discipline (highly desirable).
Current Working with Children and NDIS clearance check is mandatory.
