Community Support Worker – Aged Care & NDIS (Complex Supports)

Are you an experienced Aged Care or NDIS Support Worker looking for a new challenge within an innovative, person-centred model?

VIVA Mutual is a not-for-profit organisation supporting older adults and people with disability to live life their way through our Self-Managed Team approach.

The Role

We are seeking skilled and compassionate Community Support Workers to support Aged Care clients and NDIS participants, including individuals with complex physical supports, dementia, motor neurone disease and Positive Behaviour Support needs.

You will provide in-home and community-based support that promotes independence, dignity and choice.

Key Duties

• Personal care, daily living and domestic assistance
• Social support, companionship and community access
• Transport to appointments and activities
• Implementation of clinical and behaviour support plans
• Monitoring, documentation and reporting
• Active participation in Self-Managed Teams

About You

• Certificate III in Individual Support, Aged Care, Disability or related field (Diploma of Nursing desirable)
• Experience in aged care, disability and/or complex supports
• Strong understanding of person-centred care and compliance requirements
• Empathetic, reliable and confident working independently

Mandatory Requirements

• NDIS Worker Screening Check
• National Police Check
• Working With Children Check
• First Aid & CPR
• Manual Handling and Medication Assistance (or willingness to obtain)
• Current Driver’s Licence, access to a vehicle and willingness to travel
• Right to work in Australia
• Availability across a set roster (including 24/7 supports where required)
