About the Role
We are seeking an experienced and compassionate Care Coordinator to join our dynamic team based in Melbourne. In this hybrid role, you will be working both onsite and from home, supporting participants across Victoria and Queensland. You will play a key role in coordinating quality supports, developing individualised care plans, liaising with participants and their families, and ensuring compliance with NDIS standards.
This position is available as either full-time or part-time, with flexible working arrangements to promote work-life balance.
Key Responsibilities
Coordinate and manage a caseload of NDIS participants, ensuring their goals and needs are effectively supported.
Develop, implement, and review individual care and support plans in consultation with participants, families, and multidisciplinary teams.
Facilitate access to appropriate supports, services, and community resources.
Maintain regular communication with participants, families, support workers, allied health professionals, and relevant stakeholders.
Monitor and report on participant progress, including documenting outcomes and incidents as required.
Ensure compliance with NDIS Quality and Safeguards Commission requirements and organisational policies.
Advocate for participant choice, dignity, and rights, ensuring all supports are delivered in a culturally sensitive, respectful manner.
Support intake, onboarding, and assessment processes for new participants.
Collaborate with team members across Victoria and Queensland to ensure consistent, high-quality service delivery.
Participate in regular supervision, team meetings, and ongoing professional development.
About You
Essential:
Tertiary qualification in Disability, Social Work, Community Services, Nursing, or related field (or substantial experience in a similar role).
Demonstrated experience in care coordination, case management, or a similar client-facing role within the NDIS or community services sector.
Excellent understanding of the NDIS, person-centred practice, and relevant legislative requirements.
Exceptional communication, interpersonal, and organisational skills.
Strong problem-solving ability and a passion for empowering others.
Ability to work autonomously and as part of a remote/hybrid team.
Strong IT skills, including proficiency in case management systems and Microsoft Office Suite.
Current National Police Check, NDIS Worker Screening, and Working With Children Check (or willingness to obtain).
Current driverβs licence and access to a reliable vehicle.
Desirable:
Experience supporting clients with complex needs, mental health conditions, or high-intensity supports.
Knowledge of culturally and linguistically diverse communities.
Experience working across multiple states/jurisdictions (Victoria and Queensland).
