We are seeking a dedicated and experienced NDIS Support Coordinator (Level 2)/Psychosocial Recovery Coach to join our team on a casual basis, working remotely. This flexible role allows you to choose your availability between 3 to 5 days per week.

As a Support Coordinator, you will assist NDIS participants in implementing their plans, building capacity to manage their supports, and making the most out of their funding. Your work will directly contribute to empowering individuals to live more independently and confidently.

Key Responsibilities


Coordinate and implement NDIS plans by connecting participants with appropriate services and supports.

Build participants’ capacity to understand and manage their own plans and services independently.

Work collaboratively with participants, carers, service providers, allied health professionals, and community organizations.

Assist with resolving service delivery issues, troubleshooting barriers, and escalating matters appropriately.

Maintain up-to-date case notes and participant records using secure client management systems.

Support participants with plan reviews, goal tracking, and reporting requirements.

Promote choice and control in all aspects of service delivery.

Selection Criteria
Essential:


Demonstrated experience in NDIS Support Coordination (Level 2)/Psychosocial Recovery Coach or a similar case management role.

Thorough understanding of the NDIS framework, participant rights, and principles of person-centred support.

Excellent communication and organizational skills.

Ability to work autonomously with strong time-management and problem-solving skills.

Comfortable working with technology (Microsoft Office, CRM systems, virtual communication platforms).

Current NDIS Worker Screening Check, and Police Check.

Relevant qualifications in Social Work, Psychology, Community Services, or a related field or 3 + plus years experience.

Desirable:


Lived experience or deep understanding of disability and inclusion.

Experience working with participants with complex needs (psychosocial disability, dual diagnosis, etc.).
