NDIS Support Coordinator

Work location: The job is based in Coburg VIC 3058; however, this role involves meeting participants at their homes or other venues in metropolitan Melbourne.
Community Engagement and Innovation Network (CEIN) is dedicated to empowering individuals with disabilities to actively engage and thrive in their daily lives. Since its establishment in 2016, CEIN has focused on building capacity, promoting the development of routines, enhancing choice and control, and encouraging meaningful community engagement for all participants in the National Disability Insurance Scheme (NDIS).
We are currently seeking a dedicated and experienced NDIS Support Coordinator to join our team on a part-time basis, working three days per week, with the potential to transition to full-time in the near future. The ideal candidate will have a strong understanding of the NDIS framework, excellent communication skills, and a compassionate approach to supporting participants in achieving their goals. This role offers a rewarding opportunity to make a meaningful difference in clients' lives while working within a supportive and dynamic environment.
YOU WILL BE REQUIRED TO USE A PERSON-CENTRED APPROACH TO:
Conduct initial assessments of participants.
Increase participantsβ capacity to understand the NDIS and maximize NDIS funding.
Link participants with appropriate NDIS and mainstream services.
Conduct care team meetings for complex clients.
Maintain case notes and compile reports of participants' progress.
Develop plan review reports for the end of the participant's NDIS plans.
Establish strong rapport and working relationships with a range of stakeholders, including people with disabilities and their family members, NDIS Local Area Coordinators, NDIS Planners, community organisations, other NDIS service providers, and internal management.
WE'RE LOOKING FOR AN INDIVIDUAL WITH:
Previous experience as a Support Coordinator, Local Area Coordinator, or related role is required.
Experience working with people with all types of disabilities and support needs.
Understanding planning, case management, coordination, and reviewing for people with complex needs.
Advanced communication and interpersonal skills.
Established time management, problem-solving, and conflict management skills.
Report writing skills.
Current Working with Children Check.
First Aid certificate preferred.
Successful candidates will be required to complete:
NDIS Worker Screening Check clearance.
Working with Children Check - Employee.
NDIS Worker Orientation Module certificate.
Infection Control Module Certificate.
Provide two employment referees for reference checking.
