Who we’re looking for:
We're currently seeking both a Level 3 Specialist Support Coordinator and a Coordinator of Supports. Successful applicants will have:
A strong working knowledge of the NDIS and its interface with mainstream and government systems (State and Federal).
Experience or understanding in one or more of the following areas:
Health and NDIS interface, including complex hospital discharge planning.
Justice system and forensic support needs.
Out of Home Care (OOHC).

Qualifications & experience:
Must have an understanding of the NDIS Legislation and experience in a Support Coordination or similar role.
A bachelor degree (Social Work, Psychology, OT, Physio, RN or similar).
A solid understanding of the provider/mainstream landscape.
Drivers licence required.
Tasks & responsibilities:
Contribute to continuous improvement and positive team culture.
Supporting NDIS participants to build capacity and navigate the NDIS and other supports, while negotiating appropriate support and services;
Understanding and negotiating complex support plan’s for participants to overcome barriers in care and everyday life.
Coordinating services as defined in the participant's NDIS plan, utilising a capacity-building approach;
Preparing, implementing and reviewing service agreements and goal outcomes to ensure appropriate standards are met; Resolving crisis situations that impede a participant's ability to achieve their goals
Identifying opportunities for innovative service delivery that empowers participants to exercise choice and control in accessing a range of supports and services.
A firm understanding of local provider landscapes.
Demonstrates a strong understanding of the NDIS Act, Code of Conduct, and Practice Standards, with the ability to identify breaches and implement appropriate reporting and compliance processes when necessary.
