The Role


As an NDIS Support Coordinator, you’ll work directly with NDIS participants to navigate their plans, connect them to appropriate services, and empower them to take control of their supports. You'll be the trusted guide who helps them maximise the value of their NDIS plan and achieve their goals.

Key Responsibilities


Build and maintain meaningful relationships with NDIS participants, families, and service providers
Assist participants to understand and implement their NDIS plans
Identify, connect, and coordinate suitable services and supports
Monitor service delivery to ensure quality outcomes
Collaborate with internal and external stakeholders to support participants' goals
Maintain accurate, timely documentation in line with NDIS requirements

About You


Previous experience as an NDIS Support Coordinator or in a related
Certificate IV in Disability/Ageing Support or Diploma in Community Services (or higher)
Sound knowledge of the NDIS framework and processes
Strong communication and relationship-building skills
Self-motivated and highly organised with the ability to manage a varied caseload
Current Driver’s Licence and reliable vehicle
Valid Working with Children Check and NDIS Worker Screening Check
Experience with electronic health records or care management software is highly regarded
