We are currently seeking a passionate and organised NDIS Service Coordinator to join our growing team. The successful candidate will play a key role in coordinating NDIS services, managing rosters, and maintaining strong relationships with both participants and support staff.
Roster Management:
Develop, manage and maintain staff rosters ensuring shifts are filled appropriately and meet the individual support needs of NDIS participants.
Staff & Participant Matching:
Link the right support workers with the right participants, ensuring compatibility in terms of experience, personality, and support requirements.
Client Engagement:
Conduct monthly face-to-face meetings with participants to monitor services, gather feedback, update support preferences, and ensure goals are being met.
Communication & Coordination:
Act as the key point of contact between participants, support workers, families, and other stakeholders. Ensure clear and consistent communication across all parties.
Incident Follow-up & Service Quality:
Follow up on incidents or concerns raised by participants or staff, ensuring timely resolution and continuous improvement in service delivery.
Compliance & Documentation:
Maintain up-to-date participant files, case notes, and ensure services align with NDIS Quality and Safeguards standards.
