Your Impact
We enable our NDIS Specialist Support Coordinators to deliver exceptional care by having administrative burdens minimised. We provide a framework that streamlines processes, allowing you to focus on participant-focused care and effective plan implementation.
You will manage your own caseload, empowering individuals to achieve their goals by navigating the NDIS and connecting them with relevant supports.

Your Contribution:
Plan Implementation: Bringing NDIS plans to life in a way that aligns with participant goals and funding.
Assessment & Planning: Conducting thorough assessments and developing effective support plans.
Service Navigation: Connecting participants with appropriate and beneficial service providers.
Capacity Building: Empowering participants to grow their skills and independence.
Stakeholder Engagement: Collaborating with families, providers, and the community.
Documentation & Reporting: Maintaining accurate records and providing timely updates.
Crisis Support: Offering crucial assistance during challenging times.
Who You Are:
Minimum 2 years of Level 3 Support Coordination experience.
Proven ability to work autonomously and make sound judgments from a home-based setting.
A solid grasp of the NDIS Framework and relevant community resources.
Excellent verbal and written communication skills.
Motivation to support individuals with diverse needs (mental health, physical, cognitive) to thrive.
Sensitivity and respect for diverse cultures.
