About the role

This part-time/full-time Support Coordinator role at FULL CIRCLE SUPPORT SERVICES in Thomastown, Victoria. This is an excellent opportunity to make a meaningful impact in the aged and disability support sector. As a Support Coordinator, you will play a crucial role in empowering individuals to achieve their goals and live independently within the community.

What you'll be doing

  • Collaborating with participants to develop, implement and monitor individualised support plans
  • Coordinating and connecting participants with appropriate service providers to address their needs
  • Advocating for participants to ensure they receive the necessary support and resources
  • Monitoring the progress of participants' goals and adjusting support plans as needed
  • Maintaining accurate records and reporting on participants outcomes
  • Participating in team meetings and professional development activities

What we're looking for

  • Relevant tertiary qualification in a human services field, such as social work, psychology or disability studies
  • Demonstrated experience in a similar support coordination or case management role
  • Strong understanding of the disability support sector, including relevant legislation and funding models
  • Excellent communication and interpersonal skills, with the ability to build trust and rapport with participants
  • Problem-solving skills and the ability to think creatively to find solutions
  • Strong organisational and time management skills
  • Commitment to providing high-quality, person-centred support

KPIs

  • All approved support coordination hours are billed in accordance with the individuals NDIS support plans.
  • All hours of support coordination provided are monitored, recorded, and accounted for
  • NDIS participants service agreements in place for all Support Coordination participants
  • A range of options is provided to participants to select from to achieve the outcomes in their NDIS support plan
  • Ensure all goals identified in the participants NDIS plan are implemented, where practicable
  • All Participant NDIS support plans are reviewed as required

Our ideal candidate has:

  • Relevant tertiary qualifications in disability or a related discipline  
  • Experience within disability services or related services
  • Demonstrated service knowledge of Disability and Mental Health.
  • Demonstrated methods that promote engaging individuals with a disability with their community and a commitment to principles involved in community inclusion;
  • Demonstrated time management in fast-paced, complex service environment;
  • Ability to use all relevant technology including tools enabling mobility and work autonomously.
  • Current Drivers License
  • Working with Children Check
