What You’ll Be Doing
You’ll be supporting NDIS participants with Level 2 and/or Specialist Level 3 coordination, empowering them to build capacity and navigate the system with confidence. More specifically:
Deliver person-centred, independent coordination that genuinely makes a difference.
Help participants understand and optimise their plans to live more freely and independently.
Coordinate services in ways that are responsive and culturally sensitive.
Travel within the Northern suburbs of Melbourne for face-to-face support when needed.
About You
You’re not just experienced – you’re passionate, proactive, and purpose-driven. You are based in the suburbs of Melbourne and bring:
A deep understanding of the NDIS and the broader community/health landscape.
Excellent communication and organisational skills.
A self-starter mindset with a love for meaningful connections.
A willingness to meet participants face-to-face.
